In an 18-month randomized, double-blind trial with 189 adults, 89.3% of the hydroxyapatite group showed no increase in decay, against 87.4% of the fluoride group. A 2025 systematic review pooling four randomized trials found no significant difference between the two for new or worsening lesions. Fluoride works by making enamel harder to dissolve. Hydroxyapatite works by putting enamel back. Different route, comparable outcome.
